We value your time with us. And we understand that emergencies arise at the worst times. If you need to cancel, send an email 24 hours prior to your scheduled session. This is a courtesy to the therapist and allows enough time for the appointment to be re-booked.
If you cancel less than 24 hours of the scheduled appointment, you will be billed a $35 cancellation fee.
“No Show” Policy
A scheduled appointment is an agreement where the therapist and client prepares for this allotted time. If you can not call ahead and do not show for the scheduled appointment, you will have to pay for the full service fee.
Privacy: Information Collection, Use & Sharing
We are the sole owners of the information collected on this site. We only have access to/collect information that you voluntarily give us via email or other direct contact from you. We will not sell or rent this information to anyone.
We will use your information to respond to you, regarding the reason you contacted us. We will not share your information with any third party outside of our organization, other than as necessary to fulfill your request, (e.g. to ship an order or to consult with your referring physician).
Our security measures when collecting and submitting sensitive information both online and offline includes encryption of credit card data and a secure double lock facility, you may verify encryption by looking for a lock icon in the address bar and ‘https’ at the beginning of the address of the web page.
NO REFUND POLICY
FMM, PLLC has a no refund policy for medical massage treatments, wellness memberships and body contouring services. These services are prescribed services with a set amount of sessions, Annual Member plans and results-oriented precise cumulative body contouring sessions that require upfront payment. A select few of the treatments have payment installments to allow patients to afford the care without financial hardship. The patient or client is presented with a treatment plan acknowledgment form prior to any payment which outlines no refund and patient responsibility to pay upfront.
